Forms for Membership

To join AFCU, complete a Membership Application. Be sure to include address of your sponsor group whether a family member or employer and all phone numbers for us to have on record.

If you want to start a payroll deduction/direct deposit to come into your account(s) at the credit union, complete the Payroll Form. Complete the Member Information and Payroll Authorization for payroll to be deposited into your savings account. Complete the entire form if you want that deduction/direct deposit transferred to your checking account, loans, or other accounts.

Beneficiary Card - The primary benefit for the member who has an individual membership (single ownership) with a Payable Upon Death Agreement is the ability to designate a beneficiary of funds without the lengthy probate process. Send all of the above including a photocopy of your government issued photo ID to Arkansas Federal Credit Union, Attn: Marketing Dept., PO Box 9, Jacksonville, AR 72078-0009. Please include a note if you have special instructions: order checks, order ATM/VISA Check Card, etc.
