AFCU Downloadable PDF Forms
AFCU has numerous forms available for download in PDF format for you to print and complete in the comfort of your home or office.
Simply click on the form you need, and the form will appear in a pop up window in PDF format. You may then print the form or save it to your computer for use at a later time.

Subordination Request Form
Click on Subordination Request Form to print a form to request a subordination agreement.This form is used to make the request to subordinate the lien on your home equity loan.
Membership Application
Click on Membership Application to print a membership application form. Use this form if you are a new member joining AFCU or you would like to add additional individuals to your account.
Payroll Form
Click on Payroll Form to print a payroll authorization form. Use this form to start or change a payroll deduction or direct deposit into your AFCU account(s). You may also use your employer’s direct deposit form.
Deposit Slip
Click on Deposit Slip if you would like to mail a deposit to AFCU. Our mailing address is P. O. Box 9, Jacksonville, AR 72078-0009. You may also print and complete a deposit slip prior to visiting our branch locations or drive thru to expedite your transaction.
Payable Upon Death/Beneficiary Card
Click on Payable Upon Death if you would like to designate a beneficiary for your funds without the lengthy probate process.
Wire Transfer
Click on Wire Transfer you would like to send a wire transfer from your account. You may complete the form and fax it to our Call Center at 501.982.3299. Faxes received after 3:00 p.m. are processed next business day.
Address Change Request Form
Click on Address Change if you would like to update AFCU with you new mailing address.
Business Partner Request Letter
If your business or association would like to offer AFCU membership as an option to your employees or association members, simply copy the Business Partner Request Letter onto your letterhead, fill in the requested information, and mail, fax, or e-mail the letter along with the Employer/Association Profile to the Marketing Department at AFCU.

There are four different letter options based on your number of employees and the distance from your office to the nearest AFCU branch location.
- Less than 3,000 employees, Less than 25 miles from an AFCU branch
- Less than 3,000 employees, More than 25 miles from an AFCU branch
- More than 3,000 employees, Less than 25 miles from an AFCU branch
- More than 3,000 employees, More than 25 miles from an AFCU branch
Employer/Association Profile
If you are interested in offering AFCU membership and all our financial products and services as an option to your employees or association members, please print an Employer/Association Profile and fax it to the Marketing Department at 501.982.3299.
‘‘I Want AFCU Letter’’
Credit Unions differ from other financial institutions in that you must qualify for membership in a credit union as opposed to other financial institutions being open to the public. AFCU is not available to the general public, hence the membership. When you belong to a credit union, you are a member/owner instead of a customer.
To join AFCU you must be employed by, a member of or volunteer with a company or association that offers AFCU as a benefit. You may also join AFCU if you have a family member who is eligible to join AFCU (The family member does not have to be a member of AFCU but must be eligible to join based on their employment or association membership).
If your current employer or association does not offer you the option to join AFCU print an "I Want AFCU Letter". Give this letter to your Human Resources department or Benefits Personnel and tell them you would like the option to join AFCU! We will be happy to send them an information packet to look over at their convenience.
